The Mission Change revisions to the Form 33, Injection Well Permit Application have been released in the eForm system. Form 33s in “Draft” are converted to the new version. Form 33s “In Process” will stay in the current version unless returned to “Draft.” If returned to draft, they will be converted to the new version.
