ECMC has launched an update to the Form 5A. It includes a section for “as-completed” Top of Production Zone location reporting. The new fields are only seen on Form 5As that have been created after 5/13/2025. All Form 5As in Draft, and In Process will remain in the old version and will not display the new fields.
