The Mission Change revisions to the Form 26, Source of Produced Water for Disposal have been released in the eForm system. Form 26s in “Draft” are converted to the new version. Form 26s “In Process” filed prior to Jan. 15, 2021 stay in the current version unless returned to “Draft.” (If returned to draft, they will be converted to the new version.) The Form Instructions for COGCC Forms 1 and 1A have been revised with updated contact information. COGCC Financial Assurance Supervisor Debbie Lutz and Field Operations Financial Liaison Deb Bracey are the contacts for Forms 1 and 1A.
The Operator Guidance for Informed Consent is now available on the Form 2A information page. Two different informed consent template letters are also available on the Form 2A page: one is a Statewide Template for use outside of Weld County, and the other is the Weld County Template.

As part of the Mission Change Rulemaking (effective January 15, 2021), Rule 313 and Rule 313A were removed thereby eliminating the requirement to file the Form 11, Monthly Report of Gasoline or Other Extraction Plant.
